Treatment Terms & Conditions

Your Care. You are under the clinical care of your consultant who may involve other doctors in treatment if appropriate. One Heart Clinic staff provide care and treatment under the consultant’s clinical instructions. Consultants are not employees of One Heart Clinic but work with us under practicing privileges. Your consultant is responsible for arranging your consent for specific medical or surgical treatment if it is required.

The Law. The contract to deliver treatment is between One Heart Clinic (providing the services) and the patient. The contract shall be governed by and construed in accordance with English law and English courts shall have exclusive jurisdiction. The patient’s or patient’s representative signature on your registration form confirms acceptance of the terms and conditions.

Third Parties. Except as expressly provided elsewhere in these terms and conditions, a person who is not a party to this agreement shall not have any rights under the Contracts (Rights of Third Parties) Act 1999 to enforce any term of this agreement.

Payment Liability You are responsible for approving the treatment with your insurer or sponsor, and providing all necessary information to enable direct billing arrangements. You are liable for all personal expenses, settlements, and charges not settled by your insurer or sponsor. You consent to your card payment details being held on file by One Heart Clinic and accept that One Heart Clinic may debit this card with prior notification if any shortfall remains unpaid. Payment is required in full before or at the time of treatment or consultation if you are self-funding your care.

Late Payment and Debt Collection If for any reason payment is not received by the specified due date, we reserve the right to forward the outstanding invoice to a third-party debt collection agency for legal collection. If your invoice is sent for legal collection, you acknowledge that we may add debt collection fees to the total amount owed. These fees will be outlined in the invoice sent by the third party. Once your invoice has been forwarded for collection, you agree to communicate and resolve any outstanding debts directly with the third party.

Your Health Record. A summary of your health record will be sent to your GP after your discharge from the clinic unless you request this not to be shared. Your consent is deemed given unless indicated in writing.

Clinical Audit. The clinic is responsible to audit clinical records for quality assurance reasons, which may include your health record. Your consent is deemed given unless indicated to us in writing. Your data will be treated strictly in line with National CQC Standards and will be subject to confidentiality according to the Data Protection Act.

Valuables One Heart Clinic does not accept any responsibility whatsoever for the loss of any cash or valuables belonging to patients or visitors.

Financial Interest. Some of our consultants are partners of One Heart Clinic, and have financial interests in One Heart Clinic and the equipment therein.

Membership Terms & Conditions

Scheme membership will be allocated based on your condition; you are able to choose from the following options below. Once you have paid and accepted these terms you will receive the services set out for the condition selected.

Hypertension Membership

The tests will be booked once a year and delivered from your choice of our clinic locations.

Valve Membership

The tests will be booked once a year and delivered from your choice of our clinic locations.

Other Inclusions

All the above membership options include access to the following general services:

General

Once you have registered and paid for your scheme you will be contacted by One Heart Clinic to book the relevant tests for your membership type. You will be able to access advice via email with your named consultant via their PA up to 4 times during the membership period.

One Heart Clinic reserves the right to withdraw membership and suggest alternative care pathways for you if the level of support is deemed beyond reasonable or you require a service we do not provide.

The standard appointment duration with your consultant is 30 minutes and will be delivered either face to face or by video consultation at your request.

We will provide a private prescription for any drugs that you may require, there is no additional charge for the prescription, but you will need to pay for the medication. We will also provide up to 8 prescription amendments per scheme year at no charge.

We will use our reasonable endeavours to ensure that you will be able to see your consultant at your preferred date and time.

Please note that if you fail to attend the booked appointment for your annual consultation or applicable tests, or cancel with less than 24 hours’ notice, we reserve the right to charge you an additional fee for rescheduling any or all of the missed services.

Payments

Payment can be made annually by debit or by credit card or monthly. Monthly payments will be processed on the date you subscribe, and then monthly thereafter on that date.

You are entitled to cancel your membership and receive a full refund of fees paid within 14 days from the date of registration and initial payment provided that you have not used any of the Services.

The initial membership period is 12 months, and we cannot offer a refund during this period if you cancel your membership once the 14-day period has passed or if you have used any of the Services.

If you use any services, not covered by the membership benefits, you must pay for these in full at the end of each appointment. You will be fully informed of any costs not covered by the scheme. An invoice and payment receipt will be provided on request.

All members are entitled to a discount of 10% from our published self-pay prices for any further tests outside of scheme.

Your membership is not transferable to another person.

Data Protection Notice

Confidentiality. The confidentiality of patient information is of paramount concern to One Heart Clinic. To this end, One Heart Clinic fully complies with Data Protection Legislation and Medical Confidentiality Guidelines. One Heart Clinic sometimes uses third parties to process data on its behalf. Such processing, which may be outside of the EEA, is subject to contractual restrictions with regards to confidentiality and security in addition to the obligations imposed by the Data Protection Act 2018.

Medical Information. Medical information will be kept confidential. It will only be disclosed to those involved with your treatment or care, including your GP, and, if applicable, to any person or organisation who may be responsible for meeting your treatment expenses, or their agents.

Fraud. Information may be disclosed to others with a view to preventing fraud.
